Nicole Beauty and Wellness Expert

THE CONTRACT will be signed by the person booking services for a particular session, and will specify the location, date, time, number of persons receiving makeup service, and what time each person will have reserved. If services for more than one person are being booked, all deposits must be paid at the time of contract signing to guarantee each reservation. Due to the possibility of schedule conflicts, additional makeup applications added after contract signing or changes made to the location, date, and time of event will be done at the sole discretion of Nicole Perkins. 

This contract is the only agreement between the person(s) booking / contracting the services of Nicole Beauty and Wellness Expert [hereafter referred to as Client(s)] and Nicole Perkins
(Professional Makeup Artist.)

*** Once you have signed the contract with the current rate, you are guaranteed that rate if the price increases.***

PAYMENT. In full consideration for all services rendered by Nicole Perkins, the client agrees to make the following payment in the U.S. funds to the artist (Nicole Perkins). 

A) BRIDAL TRIAL. A non refundable fee of $125 is due prior to the specified day and time of the consultation.

B) WEDDING DAY DEPOSIT. 50% of the final balance must be paid to secure event day and time unless otherwise specified. 

C) PAYMENT OF BALANCE. The final balance agreed upon at the initial signing of the contract is due on the day of event- no exceptions. The person(s) responsible for the entire payment is the person(s) who has signed the booking contract. Please understand that even if the wedding party size decreases, client is responsible for the agreed upon amount set forth by client and Nicole Perkins.

D) Touch-Ups: $100 per hour (pertains to client(s) if needed for touch-ups after the initial application). 

E) Acceptable forms of payment are: Credit/Debit card, cash, and money order made payable to NICOLE PERKINS. 

F) COVID-19. Re-scheduling due to the COVID-19 pandemic is allowed at no risk of losing your deposit provided Nicole Perkins is available for the new date. This is valid for sudden change of state or venue guidelines, sickness or recent exposure of bride, groom, or artist (Nicole Perkins). There are no refunds, only re-scheduling for COVID-19 related changes. Booking a new date is subject to artist's (Nicole Perkins) availability. 
*****DELAYS ON THE WEDDING DAY*****
Bridal parties are expected to arrive at or before the time that is allotted for the day of event. Late appointments WILL NOT be accepted on the day of the wedding.

SATISFACTION GUARANTEED: Makeup will be completed to client's satisfaction, but is not to exceed allotted makeup time. Ample time is given for each makeup application upon booking. Acceptance of the makeup application by client is acknowledgement by client that makeup is done to his/her satisfaction. 

CANCELLATION: Deposits and final balances are non refundable and non transferable. If Nicole Perkins cancels a full refund will be issued electronically or by money order within (30) days.

MISCELLANEOUS. The Makeup Service Agreement and the attached hereto rider(s), if any, set forth the entire agreement between the parties, and may not be amended except in writing signed by both parties. The Makeup Service Agreement will be governed by and constructed in accordance with laws of the state of Missouri, without regard to the principles of conflicts of law. In any action or proceedings involving a dispute between the Client and Nicole Perkins arising out of this Makeup Service Agreement, the prevailing party will be entitled to receive from the losing party reasonable attorney fees. 

LIABILITY: Nicole Beauty and Wellness Expert is acting as a non-exclusive independent contractor for the client. Each vendor is responsible for their own actions and enforcement of their own contract (if applicable). Nicole Perkins will not be responsible for any type of allergy, breakout, or discomfort from makeup application. All brushes, implements, and makeup products are kept sanitary and are sanitized after every application. Makeup products are hypoallergenic. Any skin condition should be reported by the client to the makeup artist prior to application and, if need be a sample test of makeup may be performed on the skin to test reaction. Client(s) agrees to release the makeup artist, Nikki Beauty and Wellness Expert, it's owners, and all employees and agents from all liability for any skin complications due to allergic reaction. 

ON LOCATION SERVICES: For weddings in a 30 mile radius of downtown St. Louis mileage is free. After 30 miles a mileage charge of $1 per mile will be added to the final bill. All parking fees, toll fees, and valet fees will be added to the final bill. 

AIRFARE AND ACCOMODATIONS: All costs for travel to a booked event are to be paid by the client. Costs are may include, but are not limited to: airfare, hotel, transportation, parking, service incidentals and all taxes. 

USE OF IMAGE: Nicole Perkins may use on its website - and/or on any brochure, flyer or other advertising it deems necessary - any and all photographs, video, audio, and any digitally or chemically stored media that is captured or recorded by Nicole Beauty and Wellness Expert and any of it's employees, representatives, or agents. Client(s) agree to release any claims regarding his/her image for such purposes.
